Olive Tree Senior Advisors assists families navigating care for older adults in Los Angeles, South Bay and the surrounding areas. Our mission is to support older adults in maintaining a high quality of life as they age and promote the ability to thrive in a safe and nurturing environment. We do this by assessing and identifying their specific needs. With that information we will recommend solutions to meet those needs to help ensure their health and safety, emotional wellness and socialization opportunities. We lift the burden from family caregivers, including those who may live far away, and provide assurance that there is someone nearby in case of an emergency. We are dedicated to preserving seniors’ dignity and sense of self as they face the challenges associated with growing older.
Our services include:
- Comprehensive assessments
- Care management, coordination and monitoring
- Patient advocacy
- Connect clients with the appropriate health care professionals and resources, as well as legal, financial and insurance specialists
- Screen, monitor and offer training for home care professionals
- Crisis response and intervention
- Coordinate transitions from home to health care facilities and back to home
- Monitor progress when someone is hospitalized or in a rehab facility
- Evaluation and recommendation of appropriate senior living communities based on individual needs and preferences
- Relocation assistance and monitoring for those moving into a senior care community
- Identify and coordinate meaningful daily activities, including for those living with memory loss
- Assistance with review of potential state and federal benefits
- Provide support to family members and other care providers who are caring for an elder loved one for families who live close or far away
- Meet regularly with families to assist with decisions about current and future care needs
- After-hours and emergency assistance and support
